    I do hereby authorize ADS Background Screening, LLC to make any inquiries necessary to perform a background check on me. I understand that the results of this background check will be provided to my employer/landlord, and that my employer/landlord will provide a copy to me as well if requested to do so.

    I release ADS Background Screening, LLC and it's agents from any and all liability, including acts of negligence by ADS Background Screening, LLC, and responsiblity, damages and claims of any kind whatsoever arising from the disclosure of my background. I specifically waive any confidential relationship of privacy, which may exist and completely release ADS Background Screening, LLC. Furthermore, I understand and agree that ADS Background Screening may request my information from various federal, state, and other agencies, including public and private sources which maintain records concerning my past activities relating to my driving record, criminal record, civil matters, previous employment, educational background, and other past experiences.

    I agree and understand that a photocopy of this authorization may serve as an original.

  • Expectations
    With excellence in mind, the following is a list of expectations for each member of the student ministries team:

    1. Maintain a healthy devotional life. A daily walk with God is the key to an effective ministry.
    2. Live a life worthy of your calling…” (Ephesians 4:1). Strive to walk with integrity and purity of heart every day.
    3. Punctuality is vital to the success of all events and services. Please be on time for services. Team members should arrive 15 minutes early to services to prepare for the service.
    4. Attend at least 75% of Sunday morning services at Encounter Church.
    5. Be respectful of leadership and other ministry team members.
    6. Be prepared for service. Ministry leaders are vital to encouraging the students to grow deeper in their walk with Christ.
    7. Membership to Encounter Church is encouraged, but not required.


    Stance on Alcohol
    As an Assemblies of God church, we stand by the official position paper released by the Assemblies of God regarding the consumption of alcohol.

    All members of the student ministries team are encouraged to read this stance to better understand our position. In light of this stance, these are the following requirements for those who serve on the student ministries team:

    1. Consumption of alcohol, especially with the intent to get drunk, is strongly discouraged. Those who “socially drink” or “drink in moderation” are encouraged to prayerfully reconsider since they could potentially cause other believers to stumble into the sin of drunkenness and the addiction of alcoholism.
    2. Posting about consumption of alcohol on social media is strictly prohibited.
    3. All ministry leaders involved in small group or stage ministry must abstain from alcohol consumption.

    Dress Code
    If you are involved with the stage ministry of the student ministries team, please adhere to the following dress code:

    1. Please make sure that what you wear on stage isn’t too tight, revealing, short (above the knee is considered too “short”), sleeveless, or distracting in any way.
